When you have a disabling condition or a chronic illness, a traditional doctor/patient relationship can’t fill all of your needs. You require medical care, of course, but you also need services that help you keep your life on track. You want a process that connects you with the services and supports you need but also helps you make the most of the ones you already have. Independence is also important, so you prefer choices that allow you to maintain control over your own life.

We tailor LTSS to your specific lifestyle goals and healthcare requirements, and your desire to function on your own terms.

As with every service we provide, our LTSS strategy focuses on you. Our staff discusses your personal living situation and reviews your existing services and supports. Whether you need bathing assistance, meal delivery, transportation, or other services, we help you build a support system that’s a good fit for you and your family. We personalize your LTSS to improve your quality of life.

Person-Centered Long-Term Assistance



At Care Alliance of Western Massachusetts, when we develop an LTSS plan, we look at the big picture. We want to establish a support system that serves our clients now and in the future. We accomplish this by working with public and private partners to make sure our clients have access to quality long-term assistance and the funding to make it happen.
