Care Management Programs at BHN

BHN’s Care Management programs provide team based comprehensive medical and behavioral Care Management services for children, youth and adults in our service area.
- BHN serves individuals with physical health/chronic disease needs, behavioral health needs, and complex, co-morbid physical and behavioral health needs.
- Care Management teams are staffed by experienced Registered Nurses, master’s-level behavioral health clinicians, Care Coordinators and paraprofessional community health workers.
- Care Management teams are integrated with primary care staff in community and health centers and in primary care settings that have physical health CM nurses.
- BHN contracts with insurers such as Commonwealth Care Alliance (One Care) and the Massachusetts Behavioral Health Partnership (MBHP) to provide both medical and behavioral health care management.

BHN has assumed operations of six substance use disorder (SUD) treatment programs in Somerville, Brockton, Attleboro, Hyannis, Lawrence, and Haverhill MA. These centers continue to offer essential services including medication for opioid use disorder, counseling and case management to individuals currently in care and anyone in the surrounding communities seeking support for opioid use disorder. To mark this important milestone, BHN is hosting a series of Grand Opening Celebrations.
