Holyoke Community Crisis Stabilization (CCS)
Holyoke Community Crisis Stabilization (CCS)
Description

BHN Community Crisis Stabilization (CCS) offers a short-term, out-of-home placement to stabilize individuals in an emerging crisis situation. The program provides a home-like, safe, and supervised environment for psychiatric crisis or hospital step-down. CCS is a voluntary, unlocked, community-based treatment program.

CCS is staffed 24-hours, 7-days a week. Services include clinical assessment, medication evaluation, and the development of personal recovery tools. After-care arrangements for continued support in the community is a part of discharge planning. BHN strives to serve individuals in emotional crisis, diverting the need for the emergency room. CCS also offers step-down capacity to offer continued stabilization as a transition from hospital to home.

Our team includes a Medication Prescriber, Nurse, Clinician, Counselors, and Peer Support Specialists to provide assessment, medication management, social work, and recovery skill development.  

Referrals can be made by an Emergency Service Provider Evaluation Team or a by a Social Worker from an in-patient psychiatric setting. If you are experiencing a crisis call (413) 733-6661.

Mount Tom Center for Mental Health and recovery is a community-based outpatient mental health and substance abuse program providing on-site and outreach services in the following areas: child and adolescent, adult (general referrals and program based), developmentally delayed/mental health, substance abuse and dual diagnosis (SA/MH).
