The Behavior Management Monitor works with a child or youth (under the age of 21) in the youth’s home or other natural setting to enhance youth strengths and to reduce or extinguish problematic behaviors determined to be interfering with the youth’s optimal level of functioning. The Monitor is responsible for assisting in the implementation of a focused behavior plan developed by the In-Home Behavioral Therapist.provide daily documentation of his/her interactions with the identified service participant specific to the goal area for which Behavior Therapy is authorized and all contacts with collateral sources. The Behavior Management Monitor may participate in ICP meetings or IEP meetings as appropriate, in order to report on and promote progress in meeting identified behavioral goals. Behavior Management Monitor maintains regular contact with the In-Home Behavioral Therapist regarding his/her progress assisting the youth and family in following the established behavior intervention plan.

QUALIFICATIONS DESIRED: |
- Bachelor’s degree in a human service field from an accredited university, and one year of relevant experience working with families, children or adolescents who require behavior management OR an associate’s degree and a minimum of two years of relevant experience working with families, children or adolescents who require behavior management

The Therapeutic Mentor works with a child or youth (under the age of 21) in the community. The purpose of the Mentoring service is to enhance youth strengths and/or address areas of need, which have been identified by a behavioral health provider in a “clinical hub” service (In-Home Therapy, Intensive Care Coordination, or Outpatient Therapy.) The Mentor offers structured one-to-one strength-based services to the identified child or youth. Position Summary:
Under the supervision and direction of a Senior Care Coordinator, the Care Coordinator is responsible for comprehensive, strengths-based assessment; facilitation of Care Planning Teams; service planning and implementation; and, transition planning for MassHealth-enrolled children and youth under the age of 21 with serious emotional disturbance (SED).
Primary responsibilities:
- Conducts a strengths-based assessment inclusive of the Child and Adolescent Needs and Strengths (CANS-MA version), in conjunction with a comprehensive assessment and other clinical information to organize and guide development of an Individual Care Plan (ICP) and a risk management/safety plan. The assessment process determines the needs of the youth for any medical, educational, social, therapeutic, or other services.
- Works with each family to identify and convene the Care Planning Team (CPT) to develop an Individual Care Plan (ICP), and other relevant plans such as a Risk Management/Safety Plan.
- Facilitates Care Planning Team (CPT) meetings including reviews of the ICP, convening the CPT as needed to update the plan of care to reflect the changing needs of the youth and family (whether services are being provided in accordance with the ICP, whether services in the ICP are adequate, and whether these are changes in the needs or status of the youth and if so, adjusting the plan of care accordingly).
- Works directly with the youth, the family, and others to identify strengths and needs of the youth and family, and strategies for meeting their needs.
- Works directly with the youth and family to implement elements of the ICP.
- Prepares, monitors, and modifies the ICP in concert with the Care Planning Team.
- Identifies and actively assists the youth and family to obtain and monitor the delivery of available services including medical, educational, social, therapeutic, or other services.
- Collaborates and coordinates other services with state agencies (if involved), the school system, primary care, and other service providers on the behalf of the youth and family and as indicated on the ICP.
- Fosters child and family participation in the development and understanding of the ICP goals and interventions.
- Develops with the CPT a transition plan when the youth has achieved goals of the ICP.
- Provides after hours phone support to address urgent child or family crises. Arrange or provide home visit support services after-hours as needed to address urgent child or family crises.
- Maintain regular and consistent communication with youth, family, and Care Plan Team members and convene Care Plan Team meetings.
- Participates in the development of a child’s Individual Education Plan (IEP) as needed; and facilitate integration of any Accommodation Plan with the Individual Care Plan (ICP).
- Prepares all necessary reports and documentation of services, including authorizations, Individual Care Plans (ICPs), Risk Management/Safety Plans, Comprehensive Assessments, and evaluation data.
- Performs other duties as required.
