Individual Living Support Services

BHN Liberty Street Clinic provides mental health and dual diagnosis treatment and support to adults (ages 17 and older) living in Hampden County. Programs below are funded by DMH, Community United Way and/or most third party payors.

Individual Living Support Services Include:

  • In-home Education and Assistance
  • Case Management
  • Skill Building
  • Cash Assistance
  • Language Capability-Spanish & English

Programs/Locations:

  • Franklin/Hampshire Individual Support: Hourly supports to individuals living on their own, with housemates or significant others, who live in the Franklin/Hampshire catchment area. Supports vary from one to fifteen hours per week, and focus on assistance with the day to day details of living on one’s own, including skill building and education, liaison with other professionals, employment search, benefits/ financial management, accessing generic services, and general case management.

  • Holyoke/Chicopee Individual Support: Funded by DMR, this program provides hourly supports to 35 adults with mental retardation living on their own inthe Holyoke/Chicopee area. Supports vary from one to fifteen hours per week and focus on assistance with the day-to-day details of living on one’s own, including skill building and education, liaison with other professionals, employment search, benefits/financial management, accessing generic services, and general case management.
